阿里山森林鐵路嘉義線37K出軌事故臨時檢查辦理紀要 Summary of the Provisional Inspection on the Derailment Accident at 37K of Chiayi Branch, Alishan Forest Railway
臨時檢查現場勘查。

本局配合交通部鐵路營運監理小組於103年12月19日辦理阿里山森林鐵路嘉義線37K出軌事故臨時檢查作業。本次事故屬鐵路行車規則第122-1條規定之重大行車事故,爰依據鐵路法第41條及第56條之5規定辦理,並聘請專家協助調查事故之發生經過及其發生原因。
阿里山森林鐵路管理處於11月21日召開行車事故責任鑑定委員會會議調查及鑑定事故發生原因,造成本次出軌事故主要原因為辦理路線曲線整正及砸道工作未能完成養護程序及檢核軌道超高,導致該路段道碴量不足路基鬆軟,機關車進入曲線段時造成車輪出軌。
本次檢查作業由鐵路營運監理小組陳副總工程司景池率相關同仁及兩位外聘委員赴嘉義線37K出軌處現勘,並進行文件檢視、人員洽詢。查察內容包括巡檢、檢查及保養等紀錄、行車事故紀錄以及出軌事故相關調查報告與改善作為等。
本次臨時檢查作業於當日下午順利完成,初步調查結果與森鐵管理處鑑定委員會之鑑定結果一致。後續將撰擬調查報告並依法提出應行改進事項,簽報交通部核可後函請農委會林務局及臺鐵阿里山森鐵管理處辦理改善。

In cooperation with MOTC Railways Supervision Task Force, BOHSR conducted the provisional inspection on Dec. 19, 2014 regarding the Alishan Forest Railway derailment accident at 37K of Chiayi Branch. Due to categorization of this accident as a "major operation accident" according to Article 122-1 of Regulation of Railway Operation, the incident was investigated under Article 41 and 56-5 of Railway Law. In addition, railway experts were also invited to assist on this case and find its cause.
Alishan Forest Railway Administration Office held the Rail Accident Investigation Committee Meeting on Nov. 21 to identify the cause of the accident. The root of this accident was traced to incomplete curve adjusting and tamping maintenance procedures which caused over-height of railway. This resulted in insufficient ballast and soft trackbed, which caused the Rolling stock wheels to derail when entering the curve.
Railway Supervision Task Force Deputy Chief Engineer G. C. Chen and two other external members led the examination team to the 37K derailment site for onsite inspection, documentation examination and interviews with personnel. The investigation contents covered patrol records, examination and maintenance records, operational accident records, derailment accident data and improvement measures.
The provisional inspection came to an end in the evening in which the preliminary results were in consistence with appraisal of the Rail Accident Investigation Committee. The investigation report will be generated subsequently and will be submitted to Forestry Bureau and Alishan Forest Railway Administration Office for future improvement.
